 V3 certificate extensions for Pike v7.6 release 112
The routine for generating PKCS Certificate Signing Requests only
supported version 1 of the standard.

This lets it generate version 3 certificates.

If I'd had time I'd have added support for UniqueIds from Version 2, but
that isn't part of my requirement at the moment, so it'll have to wait
(probably indefinately).
