Hi.

As far as I can see, FIRST_TRY modules are run for all requests that
enter the normal request handling, so the only way for them not to be
called is for stuff to be sent directly from the protocol cache.

You can check if this is the case by adding $cache-status to the
logging format.

BTW: You may also want to look into using the $xff format instead of
     $ip-number in your logging pattern.
